Patient Positioning Systems for the Operating Room

This collection covers surgical patient positioning: shoulder suspension and traction systems, ankle distraction, knee and head positioners, perineal support and protective boots. These are the devices that hold a patient in the position a procedure requires, for as long as it takes, without the position itself causing harm.

Positioning gets less attention than the instruments used in a procedure, and the reason is that when it works nobody notices it. When it fails, the consequences appear hours later and away from the operative site: a pressure injury over a heel, a nerve palsy in a limb that was never operated on, a shoulder strain from an arm positioned beyond its comfortable range.

An anaesthetised patient cannot report discomfort, cannot shift their weight, and has lost the muscle tone that normally protects joints at the end of range. Every protective mechanism a conscious person has is absent, which is why positioning equipment is a safety category rather than a convenience one.

Shoulder Positioning and Suspension

Shoulder procedures need the joint held in a specific position and often under traction, with the arm suspended so the surgeon has access from multiple angles without an assistant holding it for the duration.

The reason suspension systems exist rather than manual holding is consistency. An assistant holding an arm for two hours tires, and as they tire the position drifts. Arthroscopic work depends on maintaining the joint space that traction creates, and a position that drifts closes that space at exactly the moment the surgeon needs it.

Options: the shoulder suspension kit, the McPull shoulder kit and the TBC Triton shoulder kit.

A note on traction force

Traction opens the joint space, but excessive or prolonged traction is associated with neurological complications, since the brachial plexus runs through the region being distracted. Traction weight and duration are set by the operating surgeon against the procedure and the patient, and they are monitored rather than set and forgotten.


Lower Limb Positioning

Knee

The De Mayo knee pad holds the leg at the flexion angle a knee procedure requires and maintains it without an assistant. In total knee arthroplasty the flexion angle determines exposure, and an angle that drifts during the procedure changes what the surgeon can see and reach.

Ankle distraction

The ankle distraction kit applies controlled traction across the ankle to open the joint space for arthroscopic access. The ankle is a tight joint with little natural space, so distraction is what makes arthroscopy possible there at all rather than being an aid to it.

Perineal post

The perineal post roll provides the padded counter-traction surface used in hip arthroscopy and fracture table work. This is one of the most safety-critical items in positioning: the post resists the traction applied to the limb, so the patient's perineum bears that counter-force for the duration of the procedure.

Pudendal nerve injury from perineal post pressure is a recognised complication, and post padding, diameter and positioning are among the factors discussed in relation to it. Padding here is not comfort, it is the interface distributing a sustained force across tissue that has a nerve running through it. Post selection and traction management are decisions for the operating surgeon.


Head Positioning and Airway Protection

The universal head positioner supports the head in the required position while protecting the face, and in prone and lateral positions it also has to leave the airway and the endotracheal tube unobstructed.

Prone positioning concentrates several risks in a small area. Pressure on the eye is associated with serious ocular complications, pressure on the nose and chin causes skin injury over long cases, and any movement of the head risks displacing the airway. A positioner has to hold the head firmly enough that it does not move while keeping load off the orbit and allowing the anaesthetist access.

Pressure Injury Prevention

The patient care boot kit protects the heels and offloads pressure during procedures where the patient is supine for an extended period.

The heel is the second most common site of pressure injury after the sacrum, and the reason is anatomical: there is very little tissue between the calcaneus and the skin, so body weight through that point concentrates over a small area with nothing to distribute it. In a conscious patient, discomfort produces a small shift every few minutes. Under anaesthesia that does not happen.

Intraoperative pressure injuries frequently present days after surgery, which means the connection to the operating room is easy to miss and the damage is done before anyone is looking for it. Why does ankle arthroscopy need distraction?

Because the ankle is a tight joint with very little natural space between the articular surfaces. Without distraction there is no room to introduce an arthroscope and instruments without damaging cartilage. Distraction is therefore what makes ankle arthroscopy possible rather than something that makes it easier.

What does a knee positioner do in joint replacement?
