TENS Units

Portable and Wireless Electrotherapy for Pain and Muscle Stimulation

TENS delivers low-voltage electrical current through skin electrodes to modulate the transmission of pain signals, while EMS delivers current intended to produce muscle contraction, and many devices in this collection do both. The range covers portable and wireless units from Mettler Electronics, Omron, iReliev and Drive Medical, plus the electrodes, lead wires, cases and safety accessories that keep them working.

The distinction between the two modes is the thing most worth getting right before buying, because it determines what the device is for. TENS aims at the sensation of pain. EMS aims at the muscle. A combo unit does both, but that does not make them interchangeable settings, and using the wrong one produces a disappointing result rather than a harmful one.

TENS or EMS? They Target Different Tissue

If a device is set to TENS and the muscle is visibly contracting, the intensity is high or the mode is wrong. If it is set to EMS and nothing contracts, the electrodes are likely not over the motor point. Those two observations diagnose most of the confusion people have in the first week with a new unit.

Wired or Wireless?

Wired units connect electrodes to the device by lead wires; wireless units place a pod directly on the body with no cable to the controller. The difference matters most for what the person does while wearing it.

Wireless removes the cable that catches on clothing, chair arms and door handles, which is the main reason people stop wearing a unit during normal activity. It also lets a single pod sit on a hard-to-reach area without routing a wire around the body. The trade-offs are battery management on the pod itself and, generally, fewer channels than a wired device offers.

Wired units are simpler, usually cheaper, and give more independent channels for treating multiple sites at once. For clinic use where the patient is seated for the session, the cable is not a problem.

Electrodes: The Consumable That Determines the Experience

Electrode pads lose adhesion and conductivity with use, and a worn pad is the single most common reason a unit that worked well stops working well. The symptom is usually misread as the device failing.

A pad that no longer makes full contact concentrates current through the area that is still touching, which feels sharp or stinging rather than the diffuse tingle the unit should produce. People respond by turning the intensity down, which reduces the effect, then conclude the device has stopped helping. Replacing the pads restores it.

Lead wires

Extra long TENS lead wires matter for placements where the electrodes sit far from where the unit is clipped, typically the lower back and legs. Standard leads force the unit to be worn close to the electrodes, which is not always where it can comfortably sit. Lead wires also fail with flexing over time, so they are a wear item rather than a permanent part.


Safety and Contraindications

Electrotherapy has genuine contraindications, and they are not a formality. The important ones concern implanted electronic devices, certain electrode placements and specific patient groups.

Broadly, electrical stimulation is contraindicated or requires specialist assessment where a person has an implanted cardiac device such as a pacemaker or defibrillator, and specific placements are avoided: over the carotid sinus in the neck, transcerebrally across the head, across the chest in a way that crosses the heart, over the abdomen or lower back in pregnancy, and over areas with impaired sensation where the person cannot report that the intensity is too high.

These are general points rather than a complete list, and the assessment belongs with the treating clinician who knows the patient. Read the instructions for use for the specific device, and confirm suitability clinically before a patient starts using one at home.

Emergency stop

The Mettler 4005 patient safety emergency switch for neurostimulators gives the patient a means of stopping stimulation immediately without reaching the controller. In a clinical setting where the patient is positioned and the device is out of reach, that matters: the person who knows the stimulation has become uncomfortable is the patient, and they need to be able to act on it.


Cases and Accessories

A TENS unit is a portable device that lives in a bag, and the electrodes, leads and unit all degrade faster loose than contained. Hard cases: the hard protective carry case for premium TENS/EMS and the case for wireless TENS/EMS. What is the difference between TENS and EMS?

TENS targets sensory nerves with the aim of modulating pain signal transmission, and the user feels a tingling or buzzing without visible muscle contraction.
