An alternating pressure mattress uses a pump to inflate and deflate air cells in a repeating cycle, so the points of the body bearing load change every few minutes rather than staying loaded continuously. That cyclical offloading is the mechanism, and it is fundamentally different from a static foam surface that simply spreads pressure more evenly.

Pressure injuries develop where tissue is compressed against bone for long enough to compromise perfusion. A patient who cannot reposition themselves keeps loading the same sacrum, the same heels, the same trochanters, hour after hour. A static surface lowers the peak pressure at those points. A powered alternating surface removes the load from them entirely on a cycle, which is a different order of intervention.

Overlay or Replacement System?

An overlay sits on top of an existing mattress. A replacement system takes the place of the mattress entirely. The choice affects therapeutic depth, bed height and rail geometry, and it is worth thinking through before ordering rather than after.

Overlays

Lower cost, quick to deploy, and reversible when the patient's needs change. The trade-off is depth: an overlay adds height to the bed, which reduces the effective height of your side rails and can create a rail gap that was not there before. It is also a shallower air column, so it offers less immersion for a heavier patient. Replacement systems

Deeper air columns, better immersion, no change to bed height and no new rail gap. This is the standard for anything beyond low-risk prevention. Depth matters: the Joerns Dermafloat LAL runs 10 inches deep at 500 lb SWL, which gives a heavier patient somewhere to sink to before bottoming out on the deck.

Alternating Pressure vs Low Air Loss: They Solve Different Problems

Alternating pressure cycles load off different areas of the body. Low air loss moves air across the skin surface to manage moisture and heat. These are separate mechanisms addressing separate risk factors, which is why many systems combine both.

Microclimate, meaning moisture and temperature at the skin, is an independent risk factor. Skin that stays damp macerates and becomes far more vulnerable to the same pressure that intact dry skin would tolerate. If your patient is incontinent, diaphoretic or on a surface that traps heat, low air loss is addressing something alternating pressure alone will not.

Lateral rotation

For patients who cannot be manually turned, or where turning requires more staff than are available, lateral rotation surfaces do the turning. Rotation is a clinical decision with its own contraindications, so it belongs in a care plan rather than as a general upgrade.

Bariatric Alternating Pressure Systems

Bariatric systems differ in three ways: wider surfaces to match bariatric bed decks, higher weight ratings, and pumps sized to fill a larger air volume without losing cycle timing. A standard pump on an oversized surface produces a cycle that is too slow to be therapeutic.

Does an alternating pressure mattress replace repositioning?

No. Powered support surfaces reduce risk; they do not eliminate the need for a repositioning schedule, regular skin inspection and the rest of the prevention protocol. Treating a mattress as a substitute for turning is a common and consequential error. Follow your facility's protocol and the treating clinician's plan.

What is lateral rotation and when is it used?

What happens during a power outage?

Most systems have a static or CPR mode that holds the surface inflated when the pump stops, but this is not therapy and it is not indefinite. Are replacement covers available?
